Job Description
The Care Manager leads the North Zone Complex Care Support Team, an interdisciplinary healthcare team providing specialized consultation services for clients in Continuing Care where behavioural needs cannot be met through usual care levels. Responsibilities include leadership, stakeholder engagement, team development, change management, education, complex case review, clinical guidance, performance monitoring, quality improvement, and collaboration across multiple healthcare streams including Continuing Care, Addictions and Mental Health, Public Health, Acute Care, EMS, Allied Health and Social Work. The role supports evidence-based practices, advanced continuing care operations, gerontological nursing, palliative care, psychogeriatrics, case management, clinical informatics and complex behavioural management. Position based in Hinton, Alberta with hybrid virtual work and occasional North Zone travel.
