Job Description
LPN working in Home Care as part of an interprofessional health care team, providing direct nursing care to clients across the lifespan. Responsible for nursing practice, assessment, planning, implementation and evaluation of Home Care clients and caregiver/family needs in collaboration with the Registered Nurse Case Manager. Requires accredited practical nursing education, active or eligible CLHA registration and practice permit, BCLS-HCP, post-basic Medication Administration and CVAD certification, and willingness to develop wound care expertise.
