Job Description
As an Occupational Therapist (OT), provides evidence-informed occupational therapy services through Home & Community Care. Performs assessments and interventions related to functional transfers, mobility, home accessibility, pressure and wound management, cognitive screening, wheelchair and seating assessment, eating/feeding and swallowing assessments. Supports clients in achieving optimal functional abilities and independence through restorative and compensatory interventions. Collaborates with clients, families, healthcare teams and community resources while providing assessment, treatment, education, case management, referral and rehabilitation support.
